NeurofeedbackIO / Daniel Webster provides neurofeedback training using protocols developed by EEGInfo / Beemedic as well as David Kaiser’s Default Network Training, and proprietary variations. Further information can be found on www.eeginfo.com and www.kaiserneuromap.com. By engaging in neurotherapy you confirm that you have read and understood the information provided there, have been given an opportunity to ask questions regarding questions regarding the training and that these questions have been answered to your satisfaction. You understand that no representations are made as to the effectiveness or outcomes of the training, and that this overrides any potential claims made on this or any other website or information source provided.
As parent or guardian, you give consent for the client’s neurofeedback training and confirm that you are entitled to provide such consent on behalf of the client. You agree to hold harmless NeurofeedbackIO / Daniel Webster for any actual or perceived injury. You understand that you must be honest in discussing past and present circumstances, past and current drug and medication use, and any factors that likely impact your / the client’s mental health including job and personal loss.
Pricing and cancellation policy: Session pricing is valid for agreed and prepaid bookings only. There is no obligation for NeurofeedbackIO / Daniel Webster to provide services where sessions have not been paid for in advance. Sessions will be provided on a prepaid basis only and payment is strictly non-refundable. There are no refunds for services provided. NeurofeedbackIO / Daniel Webster reserve the right to change pricing at any point for services yet to be rendered, and to cancel or terminate the agreement at any point without reason or justification.
At no time and under no circumstances whatsoever shall NeurofeedbackIO / Daniel Webster, its agent, employees and subcontractors tolerate abusive, threatening, intimidating, violent, destructive, menacing or harassing behaviour from the client or any party acting on behalf of the client. For the avoidance of doubt, this includes verbal abuse, and is not limited to belittling or demeaning comments. If such behaviour does occur, we will quietly and respectfully inform the client and / or members to handle the situation. However, if the abusive behaviour does not stop, we will consider it a breach of contract, and remove ourselves from the premises immediately without providing any further services, and all remaining fees will be due immediately and on the spot, regardless of further services not commenced and without any refund of monies paid.
